Purpose of Service: Medical clearance evaluations, paperwork completion (disability/FMLA/RTW), pre/post-op documentation; final acceptance rests with employer/insurer/surgeon.
Consent to Treatment: Voluntary consent to telehealth review, paperwork completion, and provider coordination.
Telehealth Services: All services are telehealth-based; includes consent to electronic communication and acknowledgment of technology risks.

Payment & Responsibility: Payments required upfront via website; no alternative payment forms; non-payment delays or cancels services.
Refunds, Cancellations, & Withdrawal of Services:
Non-refundable once services begin.
Cancellations must be made 48 hrs prior; a $75 cancellation fee will be applied.
The MedClearer may withdraw services for uncooperative/disrespectful behavior or missing documentation.
Refunds (when approved) process in 3–7 business days.

Turnaround: Processing begins only after all documents are submitted; standard 3–5 business days; delays from incomplete info are patient’s responsibility; expedited review available for extra fee.
Delays: Natural disasters, illness, or system failures may cause unavoidable delays; patients will be notified.
