We assist with Medical Clearance for cosmetic procedures, Lab Orders, as well as forms for FMLA, short-term disability (STD), leave of absence (LOA), return-to-work (RTW), EDD, and ADA documentation.

Yes. A Telehealth appointment with Dr. Bell is required for:
Medical Clearance (for surgery or cosmetic procedures)
FMLA paperwork (Family & Medical Leave)
Short-Term Disability (STD) paperwork
Weight Loss Program enrollment
This step ensures your information is accurate and that all medical requirements are met before your paperwork is completed or treatment begins.

We’ve simplified it for you:
Medical Clearance → Needed if you’re having surgery or a cosmetic procedure. We review your labs and medical history to make sure you’re healthy enough for your procedure.
FMLA (Family & Medical Leave Act) → Paperwork that allows you to take job-protected leave from work for a health condition, recovery, or to care for a family member.
STD (Short-Term Disability) → Paperwork that lets you receive income replacement while you’re out of workfor surgery, illness, or recovery. (This often requires both pre-op and post-op appointments.)
LOA (Leave of Absence) → Time away from work that is not always covered by FMLA. Helps you take leave but may not include job protection or pay.
RTW (Return-to-Work Note) → Documentation required by your employer stating that you are cleared to safely go back to work after surgery or illness.
ADA (Americans with Disabilities Act paperwork) → Paperwork that allows for workplace accommodations if you have a condition that limits certain job functions.
EDD (State Disability Insurance) → In some states (like California), this provides state-managed income replacementwhile you’re unable to work due to illness, injury, or surgery.

Documents can be uploaded directly with your intake form. If your employer or provider will be faxing them on your behalf, please indicate this on your intake form.

Standard processing is 3–5 business days once we receive all required documents. Expedited review and processing is available for an additional fee.

Yes. FMLA is job protection, not income replacement. If you work more than one job, you may submit FMLA paperwork for multiple employers to secure leave at each workplace.
💡 Note: A separate payment will be required for each additional employer’s paperwork.

We can only complete Short-Term Disability (STD) paperwork for one employer. By law, STD is designed to provide temporary wage replacement during your recovery — it is not meant to be collected from multiple employers at the same time. Submitting STD paperwork for more than one employer would be considered “double-dipping,” which may result in denial of benefits or legal consequences.
👉 Our recommendation: Choose the employer that offers the larger leave compensation so you maximize your benefit.

No. It is Dr. Bell’s policy that a pre-op appointment must be completed prior to surgery. Additionally, most STD companies require office notes that document the medical reason for your leave. If you did not see Dr. Bell for a pre-op appointment, we cannot provide the required office notes to support your paperwork.

Yes. We can complete FMLA paperwork for your spouse if they are serving as your caregiver. They will be required to:
Complete their own intake form
Upload or fax any required documentation
Pay the applicable FMLA fee

No. While we ensure everything is accurate and complete, final approval is always determined by your employer, insurance provider, or surgical facility.

Because services begin as soon as your intake is reviewed, payments are non-refundable. Cancellations made within the required timeframe may be eligible for partial refund minus fees (see policies).
